$82 for one hour followed by $17 per 15 min ($68 per hour)

In-person Wellness check

$50 for first 30 mins then billed at $16.75 per 15 mins ($67 per hour)

Weekly Electronic wellness check 

with client/family

Free for first 30 mins then billed at $15.75 per 15 min ($63 per hour)

Monthly Maintenance Coordination
